Survey on problems obtaining access to funding for gender reassignment treatment on the NHS
All too often, trans people who approach the NHS to seek medical assistance in their gender transition find that funding is refused, delays are lengthy, or other problems make it a very difficult path to follow. We’d like to know more about the problems people face, so that we can press for improved services.
If you have encountered problems in getting treatment on the NHS, we’d be very grateful if you could take a few minutes to complete our questionnaire, and return it to Press For Change, either:

The easiest way to complete the questionnaire is probably to download the plain-text version and either print out or return it by email … but we’ve also reproduced the questions below as a form which you can fill in, in case you prefer doing it that way.
Any information you give is helpful - don’t worry if there’s a question you can’t answer completely.
Whatever you do tell us will be treated as confidential. No names will be mentioned in any reports (unless we specifically get permission first). No information will be given to any third party. Only statistics, general observations and anonymous quotes will be used in the report.
